If you are interested in purchasing a brick in memory of a child, please fill out and print the "Brick Order Form" below.

 

Mail your completed brick order form along with your check to the mailing address below.

Beginning January 2026, brick orders will be placed on the 10th of each month.  Bricks are usually delivered 2-3 weeks after being ordered and then placed by the fountain in the Madison Children's Memorial Garden at Strawberry Patch Park.

Families are notified by a letter when a brick order is received and again when the brick is placed at the garden.

 

Only one brick per child.  If a brick has already been purchased for this child we will notify you.
